Dear Beloved Patients of SkinMed Institute,
It is with mixed emotions that we announce our departure from active practice in Colorado to move closer to family. Our last day at the office will be November 16, 2023. It has been our greatest pleasure and honor to take care of each and everyone of you over the past three years, and it is not easy for us to depart from that relationship with you.
Effective immediately, SkinMed Institute will be closed to new patients. For established patients of our practice, we will be available to provide emergency care until November 16, 2023. For routine visits, you may reference the Colorado Dermatologic Society website (coloradodermatology.org) or your insurance carrier for the names of physicians in the area that can attend to your dermatologic needs. We have also provided a list of recommended dermatology practices within our geographic location on our website. We are confident to leave our patients in their exceptionally capable hands.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or to another person you designate only with your permission. If needed, please sign the records release request form located on our website and return it to our office as quickly as possible so we may transfer your records to your new doctor or release the records to you. You may email the release to info@skinmedinstitute.com or mail it to our office. Please note that we do not have the resources to duplicate, triplicate, or distribute multiple copies of your records to multiple recipients, so please choose carefully who you would like to receive your records. Until then, Skinmed will keep your records, but it would be our preference if your records were transferred to your new provider as soon as possible.
If your request for records transfer is received by October 31, 2023, records will be produced and transferred at no charge. After November 1, 2023, a nominal charge of $15 plus $0.10 per electronic page for electronic records and $18.53 for the first 10 pages, $.087 for pages 11-40, then $0.57 per page for physical copies will be required. Your records can also be downloaded immediately and at no charge to you through your patient portal.
We have deeply valued our relationship with you, our patients, greatly. Thank you for your loyalty and friendship over the years.

Geoffrey Lim, MD
Dr. Lim’s expertise and training allow our team to offer the most effective technique for treating basal cell carcinomas and squamous cell carcinomas, the two most common types of skin cancer, as well as melanoma and other high-risk skin cancers.
